Many manufacturers assume that the cooktop is installed above an oven from the same manufacturer and do not specify minimum clearances. For ventilation, you should definitely create a cutout in the rear panel of the cabinet with the required dimensions.
Regarding point 2): IKEA specifies a minimum distance of 10 cm (4 inches) between the cooktop and the edge of the countertop for the Karlby (you can view the instructions online).

IKEA-Experte26 Apr 2016 14:26KARLBY is just particleboard with a 3.5 mm (0.14 inch) thin veneer. The surface is not very durable and requires maintenance. A laminate countertop is cheaper, more robust, and easier to care for. Consider whether a continuous laminate countertop, which you can later replace with a solid wood countertop, might be the better choice for now.

IKEA expert, we were already considering whether a wooden surface – whether veneer or solid wood – is really the right choice for us. Now I’m not so sure. But let’s suppose we initially install two removable laminate panels and maybe later replace them with a Personlig panel. As far as I can see, among the removable panels only the SÄLJAN has a thickness of 3.8cm (1.5 inches), which would be necessary for installing the oven and the cooktop. Unfortunately, it is not available in the desired appearance, but we could probably live with black for now. The question remains whether everything will fit at all. Even taking into account Schmidti’s helpful tip (thanks!), I calculated 579mm (22.8 inches) for the oven plus 55mm (2.2 inches) for the cooktop minus 38mm (1.5 inches) for the countertop, which equals 596mm (23.5 inches), so it should fit, but leaves only 4mm (0.16 inches) for the ventilation gap. Sadly, the installation manual does not specify exactly how large the ventilation gap between the cooktop and oven should be. Does anyone have experience with this and can say whether this is usually sufficient?
The alternative would of course be to choose different appliances, but I would prefer to avoid that if possible – I already selected them carefully (although apparently not carefully enough regarding the size)!
