I am currently considering this. Since nowadays you can watch TV via internet connection (IPTV), I am thinking about choosing either option.

Now the question about monthly costs. Has anyone calculated this by any chance and can give a recommendation?

For the cable connection, do I have to pay an additional monthly fee for the connection on top of the cable provider’s charges for TV/phone/internet?

Best regards

Usually, it can be done afterwards as well. For a friend, it cost 600€ (about 650 USD). They just made a small hole and "shot" the line into the house. If it were me, I would and will have both done right away.

That is quite affordable where you are.
Here, Unitymedia currently charges 1,651 euros with a contract and 2,100 euros without.
The cable is installed together with other providers.
The house is set back in the second row about 25 meters (80 feet) from the street.

I can top that... 5,500 Euros for Unitymedia, although there is still nothing in my street, so I was basically the first financer for the street (by the way, this is an existing development, not a new build area).

I definitely wanted both cable and telecom connections, with TV via satellite, so I would be completely flexible. I had gotten very used to the bandwidth from Unitymedia in my rental apartment. Now I’m without broadband cable—maybe something will come later.
