What is the common practice for wastewater management in new construction?

If a toilet or bathroom is installed on the lowest floor, do you need to run the wastewater pipes beneath the slab or (due to slope requirements for drainage)?

Of course. This doesn’t depend on the lowest floor, unless you wanted to drain a toilet on an upper floor through an external wall. The entire system should, of course, also be frost-protected.
